“Leaky Gut” Isn’t One Thing — It’s a Category. Here’s Why That Changes How We Address It.

If you’ve spent any time researching digestive health, you’ve probably come across the term “leaky gut.” It gets used constantly — often as a catch-all diagnosis. But in clinical practice, that’s exactly the problem with the term. It’s not one condition. It’s an umbrella that covers several distinct patterns of intestinal permeability and digestive difficulty, each with its own mechanism and its own path back to function.

Understanding that distinction matters, because it determines how we actually intervene.

What’s Really Happening Under the Umbrella

At the center of all of these patterns is the gut barrier — a multi-layered system built from gut microbiota, a protective mucus layer, and a single sheet of epithelial cells held together by structures called tight junctions. Those tight junctions are the gatekeepers. They’re selective: built to let nutrients, ions, and fluids pass through while keeping bacteria, undigested food particles, and toxins out of the bloodstream.

When that barrier is compromised — when the junctions loosen — substances that should stay in the gut start leaking into circulation. The immune system responds. Inflammation follows. And depending on which layer of the barrier is affected and what’s driving the breakdown, the clinical picture can look very different from one person to the next.

For some people, that looks like bloating and food sensitivities. For others, it may contribute to brain fog, joint discomfort, skin reactivity, or mood changes. The connection to symptoms beyond the gut isn’t as far-fetched as it might sound: the gut and brain are in constant two-way communication through what’s known as the gut-brain axis, and roughly 90–95% of the body’s serotonin — a key neurotransmitter — is actually produced in the gut. When protein digestion is impaired, the amino acids that serve as building blocks for neurotransmitters like serotonin and dopamine may not be as available. That doesn’t mean a leaky gut directly causes brain fog — the full causal chain hasn’t been proven in clinical trials — but the mechanistic links are real, and in practice, these symptoms often travel together.

That’s why “leaky gut” as a single label undersells what’s going on. It’s a category of barrier dysfunction, not a singular diagnosis — and the factors that contribute to it (microbial imbalance, mucosal thinning, immune overactivation, and others) often co-occur and reinforce one another in ways that are easy to miss when each one is evaluated in isolation.

This Is Measurable — Not Guesswork

One of the most important things to understand is that intestinal permeability can be objectively measured. Zonulin, for example, is a protein that directly regulates the opening and closing of tight junctions in the gut lining — in fact, it’s the only known protein in the body that does this.¹ When zonulin is elevated, it signals that those junctions are loosening and the barrier is compromised. It can be measured through blood or stool testing and has been validated across multiple conditions associated with barrier dysfunction, including celiac disease, inflammatory bowel disease, and type 1 diabetes. Other biomarkers exist as well — this is an area of active research — but the point is that barrier function is assessable, not hypothetical.

This matters because it shifts the conversation from vague symptom checklists to objective data. We can measure what’s happening, track it, and use it to guide decisions.

What Drives Barrier Breakdown

A few of the better-documented drivers are worth naming, because they illustrate how different the underlying causes can be.

Chronic alcohol use has been linked to barrier dysfunction through the byproducts of ethanol metabolism. Even acute alcohol exposure can produce mucosal damage and intestinal barrier breakdown. NSAIDs — common over-the-counter painkillers like ibuprofen — are another well-studied contributor. Research in healthy individuals has demonstrated measurable intestinal barrier disruption with both short-term and long-term NSAID use, with permeability changes detectable within 24 hours of ingestion.³

At the population level, increased intestinal permeability has been consistently observed in patients with Crohn’s disease and their first-degree relatives. A landmark prospective study found that abnormal permeability was associated with a threefold increased risk of developing Crohn’s disease, even when measured years before diagnosis — suggesting that barrier dysfunction may play a role in the disease process itself, not just result from it.²

In celiac disease, gluten ingestion triggers the release of zonulin, which rapidly increases intestinal permeability. Interestingly, this zonulin response occurs in everyone who eats gluten — not just people with celiac disease. The difference is that in genetically susceptible individuals, the immune system mounts an inappropriate response to the proteins that slip through, leading to the inflammatory damage characteristic of celiac disease.¹

These examples also highlight something important: factors like low stomach acid, microbial imbalance, and immune overactivation don’t usually exist in isolation. They tend to cluster together. Low stomach acid, for instance, can set the stage for bacterial overgrowth in the small intestine, which in turn can trigger zonulin release and further compromise the barrier. The clinical value lies in recognizing these patterns as interconnected — not in trying to draw a single straight line from one cause to one effect.

The Goal Isn’t a Supplement List — It’s Corrective Nutrition

This is where the most confusion tends to live. People look for a product to “fix” leaky gut. But the actual clinical goal is what I call corrective nutrition — and it’s different from conventional supplementation in a key way.

Corrective nutrition is not about putting someone on dozens of supplements they’ll need forever. It’s a targeted, assessment-driven approach: we identify through testing and clinical evaluation which specific steps in an individual’s biochemical process need support, provide that support for a defined period, and work on the habit changes that allow the body to maintain function on its own. The goal is restoration, not dependence.

And within that approach, sequence is everything. You cannot do meaningful repair work on a gut that’s still actively inflamed — it’s like trying to renovate a house while the fire is still burning. The two phases have to happen in order.

Phase 1 — Calm the Inflammation

Before any structural repair can take hold, the gut environment has to settle. This typically means addressing microbial overgrowth, reducing the active inflammatory load in the GI tract, and correcting any factors — like insufficient stomach acid — that may be contributing to the cycle. In my clinical experience, drawing on principles from traditional Chinese herbal protocols and decades of practice, this step cannot be skipped. It’s the same logic used in any form of infection or injury management: you address the active insult before you attempt to rebuild.

Phase 2 — Repair the Structure

Once inflammation is under control, the focus shifts to rebuilding: supporting the connective tissue and cellular structures of the intestinal lining, restoring the mucus layer, and re-establishing a healthy microbial environment that can maintain the barrier long-term. Whether the tools are traditional herbs, probiotics, immunoglobulins, or targeted nutrients, the principle is the same — repair follows resolution.

Skip Phase 1 and go straight to repair, and you’re building on an unstable foundation. Stop after Phase 1 and never address repair, and progress plateaus — symptoms improve but never fully resolve, because the structural damage is still there.

Why This Matters for You

If you’ve been told you have “leaky gut” and given a generic protocol, or you’ve tried isolated supplements without lasting results, this sequencing gap is often why. The bloating-and-food-sensitivity pattern is not the same clinical picture as the brain-fog-after-meals pattern, even though both may involve intestinal permeability dysfunction — and they don’t always respond to the same first step.

The path forward starts with identifying which factors are actually at play in your case — through objective measurement, not guesswork — then following the inflammation-first, repair-second sequence with the right support at each stage. That’s corrective nutrition: not a single fix, but a process tailored to what your body actually needs.
